Comparison of conventional coagulation tests and thromboelastography for preoperatively assessing blood loss and transfusion during orthotopic liver transplantation

چکیده
Objective: An effective blood loss and transfusion requirements assessment before orthotopic liver transplantation (OLT) may improve intraoperative bleeding management during OLT. Methods: Sixty-five patients were included in this retrospective, observational study. Correlations of conventional coagulation tests (CCTs) and TEG parameters with blood loss and transfusions were determined using Pearson’s correlation analysis. The most strongly correlating CCTs and TEG parameters were selected for logistic regression and receive operating characteristic (ROC) curve analyses. Results: Prothrombin activity (PTA) showed a strong correlation with packed red blood cells (PRBCs) and fresh frozen plasma (FFP) transfusions (r=-0.838, P<0.001; r=-0.810, P<0.001, respectively). A logistic regression indicated that PTA was an independent predictor for PRBCs and FFP transfusions. The cutoff values that best predicted the transfusion threshold for PRBCs and FFP was 83.5%. Conclusion: CCTs and TEG cannot accurately assess blood loss, but PTA showed better correlations with and good performance for the prediction of PRBC and FFP transfusions during OLT.

Reduced Transfusion During OLT by POC Coagulation Management and TEG Functional Fibrinogen: A Retrospective Observational Study
UNLABELLED Patients undergoing orthotopic liver transplantation are at high risk of bleeding complications. Several Authors have shown that thromboelastography (TEG)-based coagulation management and the administration of fibrinogen concentrate reduce the need for blood transfusion. METHODS We conducted a single-center, retrospective cohort observational study (Modena Polyclinic, Italy) on 386...
متن کاملThromboelastography Use in the Perioperative Transfusion Management of a Patient with Hemophilia A Undergoing Liver Transplantation
The thrombelastogram is a method used to monitor clotting dynamics. Thrombelastography (TE) has been used to guide therapy of coagulation disorders mostly in cardiac surgery but also in liver surgery. TE is a useful tool for perioperative management of patients at risk for coagulopathy. There are several reports describing the use of the thrombelastogram in patients undergoing orthotopic liver ...
متن کاملSevere Acute Hyperkalemia during Pre-Anhepatic Stage in Cadaveric Orthotopic Liver Transplantation
A serious hazard to patients during orthotopic liver transplantation is hyperkalemia. Although the most frequent and hazardous hyperkalemia occurs immediately after reperfusion of the newly transplanted liver, morbid hyperkalemia could happen in the other phases during orthotopic liver transplantation. However, pre-anhepatic hyperkalemia during orthotopic liver transplantation is rare. This rep...
متن کاملRotation thromboelastometry detects thrombocytopenia and hypofibrinogenaemia during orthotopic liver transplantation.
BACKGROUND Orthotopic liver transplantation can be associated with haemorrhage, particularly in patients with severe liver dysfunction. We assessed the value of rotation thromboelastometry (ROTEM) to monitor coagulation in the operating theatre, its correlation with routine laboratory findings, and its ability to guide platelet (Plt) and fibrinogen (Fg) transfusion. METHODS Twenty-three patie...

BACKGROUND Although orthotopic liver transplantation (OLT) is nowadays considered standard practice at experienced centres, it can still be affected by a significant risk of massive bleeding and its related complications. Solvent/detergent plasma (S/D Plasma) has been proposed as an alternative to fresh frozen plasma (FFP) to curtail such complications. This study aimed at evaluating the effica...
